Repeatedly getting the same images to classify

I have that feeling many times but now I can tell it for sure. I have just finished classification of this image and figured out I had already made a comment to the same image before.. This should happen. Is it a known issue here?

I got the same image again.. This shouldn't happen. Is it common here?

sometimes the same image appears with a different image id (URL), for example ACH0005cy4 and ACH00054sm, but shouldn't get same id twice...
